What documents do I need to apply for an E-VISA

4. Last Six Months Bank Statement & Accounts Maintenance Letter  
5. Visa Request Letter
6. NTN  { National Tax No.}  (If Applicable )
7. Return Air Ticket  (for E-Visa only )

Your application is subject to approval by the Malaysian Immigration Department or the Malaysian Mission Office. In case of suspicion, applicants may be required to supply further documents or be called for an interview or Skype session with the Malaysia Consulate / Malaysia High Commission / Malaysian Embassy. Requirements for now are these because the Embassy is changing the requirements very often.
